In efforts to increase our level of safety and prevention in athletics, Franklin ISD requires ANNUAL PHYSICAL EXAMINATIONS FOR ALL STUDENT-ATHLETES. Annual screening of athletes is imperative because medical conditions can change from year to year and the development of subtle issues may be overlooked. The pre-participation exam is designed to help identify athletes that may be at risk for medical issues before he or she participates in a sport.
Physical examinations may be performed by your primary care physician, an orthopedic doctor, or the school physician’s. All physicals must be completed on the UIL physical form. All the UIL forms, except the medical history and the physical examination, can be completed here.
COMPLETING THE FORMS ONLINE IS HIGHLY RECOMMENDED.
All forms must be COMPLETED ONLINE or RETURNED COMPLETED by Friday, May 6, 2016 in order for your child to participate in the mass sports physical screening.
No athlete will be allowed to participate in practices, scrimmages, games or tournaments for the 2016-2017 school year until the physical, all required UIL documents, and a copy of the parent’s insurance cards (front and back) are on file with the athletic trainer.
